The spindle amplifier module A06B 6151 H075 #H580 is an αi SP 75HV module manufactured by Fanuc for the αi SP series as a 1 spindle sensor input unit. The αi SP is one of two types in the αi SP series.  The two types are the 200-V input series and the 400V input series.  The 200-V series drives a spindle motor of the 200-V input series.
